How do I change the policy owner?
Policy ownership changes can be made by downloading, completing and submitting the Ownership and/or Beneficiary Change Request Form. You can also complete this form online by using the link at my.sbli.com/forms.
You may send the completed form to us via email at records@sbli.com , fax it to 781-994-4240, or mail it to SBLI, P.O. Box 4048, Woburn, MA 01801-4048.
If the current policy owner is deceased, a certified copy of the death certificate must also be submitted along with the Form. In this case, we’ll send confirmation of the ownership change to both the prior and new owner at their address of record.
How do I change my beneficiary information?
Beneficiary information can only be changed by submitting a Beneficiary Change Form. Depending on your state of residence, you can either download or complete this form online by using the links at my.sbli.com/forms
